Form Completion: Please ensure that you fill out the required health history and intake form completely before your first visit.

No Intake Form, No Treatment Policy: If the form is not completed before your appointment, treatment will not proceed. This policy helps filter out unserious requests, ensuring a safe and effective treatment process for both patients and therapists.

  • Incomplete Forms: If the form isn't filled out, we will send you a message and a follow-up reminder, or we may call you to complete it.
  • Appointment Cancellation: If the health history and intake form is not submitted, we may cancel your appointment on the spot or require additional verification before your treatment.

Direct Billing: If you are looking for direct billing, make sure all billing requirements, such as a doctor's note or coverage details, are met. If you're unsure, please contact your insurance provider directly. Completing the health history and intake form is necessary to sign the direct billing authorization form, allowing us to process everything before your visit and prepare your file. If you cancel after we’ve billed your insurance, we will void the claim accordingly and not charge you.

For New & Existing Patients:

Clinic Hours: We are open Mondays to Sundays. However, not all available times are posted in the booking portal. We reserve some slots for patients who require long-term, consecutive treatments or as backup in case the online booking dates are full. We are also strictly by appointment basis only.

Waitlist and Unposted Dates: If the online booking dates are full, we may have extra space available. Please text us right away if you’re interested in being added to the waitlist or if you’d like to check for unposted dates. In this case, message us directly so we can find a suitable time for you.

Treatment Environment: Thrive RMT is a home-based clinic, with all treatments conducted at a residential location. However, there is a designated treatment room, offering a setup similar to that of commercial clinics.

Safety Protocols: Thrive RMT is committed to following strict infection prevention control protocols and safety procedures when booking and treating patients. We ensure that the clinic is not crowded, with treatments conducted on a one-on-one basis.

Sanitization: Every area of the clinic is thoroughly sanitized to maintain a clean and safe environment.

Insurance Fraud: Please remember that we only want straightforward billing—no insurance fraud or any illegal transactions will be tolerated.

Aesthetic Services: In this regard, please remember that our aesthetic services are not and will not be billed through any insurance company. You will only receive a regular receipt with no RMT registration number, as these services are not part of insurance coverage nor considered to be massage therapy.

Pricing: The prices posted for massage therapy services represent the "before HST" or "before tax" pricing. Therefore, the final price would be the posted price plus HST.
